Items purchased at PHYSICIANSFORMULA.COM may be returned for a 100% money-back guarantee within 14 days of purchase. For discrepancies, please contact us within 14 days of delivery. Prior to returning an item, please call our customer service at 1-800-227-0333 to get an authorization number. Customer is responsible for all costs associated with return shipping unless an exception is determined by PHYSICIANSFORMULA.COM. Our business hours are Monday through Friday from 7:30 AM-4:30 PM PST. An authorization number must accompany all returns. Refunds will be credited to the method of payment used for purchase, but shipping & handling fees are NON-REFUNDABLE.

Return Instructions

1. Log in to your account:
   1. In the "Email" field, enter your email address, and then click "Continue".
   2. In your email account, open the email sent from our store and copy the six-digit verification code included in the email.
   3. Go back to the online store, and then enter a six-digit verification code.
2. Click "Request return" for the order that you want to submit the return for.
3. If your order has more than one item, then select the items that you want to return.
4. Select a return reason and add a note for the store.
5. Click "Request return".

If your return request is approved and requires shipping, then you receive an email with shipping instructions. After the product is returned, you receive a refund.

For merchandise purchased at one of our retail partners, please return to the original point-of-purchase to determine return eligibility.
